RMCP Follow Up Services

Continued care to those in need of timely support

Ongoing Support in a Time of Need

Follow Up is a key component to effective Crisis Care. Through follow up activities, Rocky Mountain Crisis Partners is able to outreach suicidal and high risk callers, as well as those discharged from emergency department and first responder encounters, when they are embarking on a vulnerable and difficult time in their crisis.

Follow Up Focuses on:

Further assessment of the presenting problem(s)
Continued safety screening
Assistance with resource connection
Assessment and problem solving around barriers to treatment
Identification of ongoing support until further supports are established

Crisis Line Follow Up

For Crisis & Support Line callers who present with medium to high safety risk.

Hospital Follow Up

Providing telephonic follow up to recently discharged patients from partnering emergency departments who had received an evaluation for suicidal thoughts or behaviors.

Opiate Follow Up

Designed to connect individuals to programs that utilize Medication Assisted Therapy or substance use related resources specific to Opiate Issues.

Resource Follow Up

Providing telephonic follow up to ensure non-urgent resource referrals were helpful and effective to our individuals in need.

Who Makes Follow Up Calls?

Staff at RMCP conduct follow up calls once patients and callers are enrolled into a Follow Up program. Trained bachelor’s level specialists conduct follow up calls. The Triage Specialists will provide follow up calls for those individuals who have minimal safety risk, but are in need of additional support and encouragement. All patients have access to master’s level clinicians if there are immediate safety concerns. Support Line Peer Specialists also conduct follow up calls for non-urgent or high-risk follow up activities.

Research supports Follow Up

Research suggests that follow up activities may:

Reduce future incidences of behavioral health crises through collaborative action planning

Reduce unnecessary future trips to the emergency department by encouraging utilization of the crisis line and/or additional supports

Encourage follow through with discharge plans from the emergency room

Decrease future suicide attempts or instances of self harm

Improve self-report scores of depression and social isolation
